I, the undersigned,

HEREBY AUTHORIZE AND EXPRESSLY REQUEST

 

the Italian Multiple Sclerosis Society – AISM – APS/ETS to disseminate, through all its internal communication channels (including, by way of example and not limitation, the AISM website, AISM Facebook page, AISM magazines), as well as through all possible external channels (including, by way of example, journalistic, television, and print media outlets), for an unlimited period of time, without compensation and therefore free of charge:

◊  the testimonial in any form provided by me (e.g., interview, podcast, written testimony, etc.) during the event “Inclusive and             Smart Tourist Destinations: Strategies for Innovation.”

◊  photographic reproductions of my image (including edited or adapted versions) created during the event “Inclusive and Smart         Tourist Destinations: Strategies for Innovation.”

◊  video recordings of my image (including edited or adapted versions) created during the event “Inclusive and Smart Tourist               Destinations: Strategies for Innovation.”

 

The dissemination of my testimonial and/or photographic reproductions and/or video recordings of my image — in ways and contexts that do not prejudice the reputation, dignity, and decorum of the undersigned and/or of the persons mentioned by me in the testimonial — shall take place for the pursuit of AISM’s institutional purposes.

In this regard, I hereby expressly waive, on my own behalf and on behalf of my successors and assigns, any and all claims or actions — provided that the use occurs for the purposes and within the forms and limits stated above — for any financial compensation and/or damages related to the use of the above materials, in consideration of the gratuitous nature of this authorization and the charitable purposes pursued by the Organisation.

 

 

 

 

Privacy Notice pursuant to Article 13 of GDPR 679/16

 

Dear Sir/Madam,

The Italian Multiple Sclerosis Society – AISM APS/ETS, with Registered Office in Rome, Via Cavour 181/a, and National Headquarters in Genoa, Via Operai 40 (hereinafter referred to as the “Data Controller”), pursuant to Article 13 of EU Regulation No. 2016/679 (hereinafter “GDPR”), provides you with the following information regarding the processing of your personal data.

 

1. Purpose and scope of processing

Your personal data will be processed by the Data Controller for the following purposes:

  • to document, promote and publicize the event and the activities carried out by the Data Controller through testimonials, photographic reproductions and video recordings of your image;
  • for communication activities, institutional campaigns and publication/dissemination on the Data Controller’s websites, social networks and all communication channels, including periodical publications and any other media used by the Data Controller for dissemination purposes;
  • management of your participation in the event and related activities (e.g. organizational aspects, management of any payments, documentation of activities carried out, etc.);
  • sending communications regarding conferences, informational events or, more generally, AISM institutional activities.

 

The legal basis for processing for the above purposes is your consent pursuant to Articles 6(a) and 9(a) GDPR. Without consent, your personal data will not be processed; this may affect the management of the event you intend to attend. If you have provided authorization and subsequently withdraw it or object to the dissemination of your data, the Data Controller will cease further dissemination from the date the request is received. The Data Controller will certify the removal of photographic images and/or audio-video recordings subject to withdrawal. Withdrawal will not affect processing already carried out, nor cases where deletion/removal has become impossible or particularly difficult, for example where third parties have saved the material on their own devices or where it has already been published in brochures or leaflets.

 

2. Methods of processing

The following processing operations may be carried out on your personal data: collection, recording, organization, structuring, storage, adaptation or modification, extraction, consultation, use, comparison or interconnection, restriction, erasure or destruction. Your data may be processed using electronic and/or paper-based tools suitable to ensure their security and confidentiality in relation to the purposes of processing.

 

3. Data retention period

Data will be stored in the Society’s archives for the time necessary to fulfill the purposes indicated above, as well as for fiscal, accounting and administrative requirements, to document our activities, and to respond to any data recovery needs you may have.

 

4. Data disclosure

Within the limits relevant to the processing purposes described above, your data may be disclosed to third-party companies or other entities (for example suppliers, consultants, etc.) carrying out activities on behalf of the Data Controller, acting as external data processors.

Without the need for explicit consent (pursuant to Article 6(b) and (c) GDPR), the Data Controller may disclose your data to judicial authorities or to parties to whom disclosure is required by law or necessary to fulfill the aforementioned purposes.

Your data will never be disseminated or disclosed to unauthorized third parties.

 

5. Data transfer

Personal data will be stored on servers located within the European Union. Your data may be transferred to non-EU countries, always in compliance with the required security guarantees.

 

6. Nature of data provision and consequences of refusal

Providing data for the purposes referred to in section 1 is optional; however, without such provision, testimonials and/or photographic/video reproductions concerning you cannot be published through any association communication channel.

 

7. Data subject rights

As a Data Subject, you may exercise at any time the rights provided for in Articles 15–22 of the GDPR.

In particular, you have the right to:

  • request updating, rectification, integration, erasure, anonymization, or restriction of data processed unlawfully, including data no longer necessary for the purposes for which they were collected;
  • obtain information regarding the logic, methods and purposes of processing;
  • receive your Data in a structured, commonly used and machine-readable format;
  • withdraw consent previously given at any time and object, in whole or in part, to the processing of your Data;
  • lodge a complaint with the competent supervisory authority and exercise any other rights granted by applicable legislation.
